Wellness Action Plan

If you don't make any changes in your lifestyle practices, how healthy do you think you'll be five years from now? How healthy do you want to be five years from now? What would that level of health and wellness look and feel like?

Wellness is an active process, a choice, and a way of life. When we intend to be well, we make proactive choices that enhance our lives so that we feel more successful in all areas of our existence. Wellness is the part of our well-being that describes what we have, rather than something we’re lacking. So you can choose to focus enhancing your level of wellness even if you have a chronic or serious illness.

I've developed a worksheet for you to create your own Wellness Action Plan. It's designed to guide your intentions into powerful action steps. Are you ready? It takes five minutes to complete but the results can last a lifetime.

  1. State your goal or intention and WHY you want it.


  2. Are you ready to allow what you want to come into your life?


  3. How will you know you will have reached your goal? List three things that will have occurred.


  4. What small step can you take today to reach your goal?


  5. What three steps will you take in the next few weeks to continue to work toward your goal?
